Voter Registration Form
The
Voter Registration Form (GAB-131) must be completed
accurately and completely. The form should be mailed to:
City Clerk, 9. S. Broad Street, PO Box 920, Elkhorn, WI
53121-0920. You may also register in person at the City
Clerk's office or at the polls on Election Day.
Be aware that there are time restrictions for registration by
mail to be accepted. The application must be postmarked
not later than the 20th day (third Wednesday) before an
election.
Application for Absentee Ballot
If you will be unable to get to the polling place on Election
Day, you may download the
Application for Absentee
Ballot (GAB-121), complete and submit to the City Clerk prior to the election. Applications are
also available at the front desk at City Hall. You must be a
registered voter to apply.
Request by Mail
Complete the Application for Absentee Ballot form and mail it to
the City Clerk's Office at 9 S. Broad Street, PO Box 920,
Elkhorn, WI 53121-0920. The application must be
received by the clerk no later than 5:00pm on the Thursday
before the election in order for an absentee ballot to be mailed
to you.
Request in Person
You may request an Application for Absentee Ballot in person
at the City's Clerk's Office. This may be done up until
5:00pm on the day before the election.
